Academy Admission

Admission Requirements

Upon enrollment, new and old students are required to fill up the enrollment form. The student must seek the signatures of the following designated personnel on the first form:
Step 1. Principal’s Office
Step 2. Director of Student Finance
Step 3. Cashier
Step 4. DSA
Step 5. Guidance Counselor (as needed)
Step 6. Registrar

The personal data must be filled up on the second form.
The third form is for the student’s directory for accounting purposes.
The fourth form is optional when the students want to enroll on a special class like computer, typewriting, piano or voice lessons.
Summer grades for HS students with academic deficiency are needed upon enrollment.
